HOW TO APPLY FOR THE
TRINITY COUNTY INDIGENT HEALTH CARE PROGRAM

1.	Fill out the application; do not leave any blanks.
2.	Make copies of all documentation required and attach them to your application. (See the Application Requirements page for information specific to your case that should be attached to your application.)
3.	Mail your application to: Trinity County Indigent Health Care Program P. O. Box 312 Groveton, TX 75845

What happens next?

4.	Your application and documentation will be pre-screened by a caseworker. If any additional documentation is needed to make a complete application, you will be notified by mail and asked to submit the additional information within 14 days.

5.	Once your application is complete, including all required documents, we will complete the pre-screen process within 14 days and notify you by phone or mail to set up an appointment. (We reserve the right to request additional information at any time during the application or interview process.)

6.	Should your case be denied, you will be mailed a denial letter no later than 14 days from the determination date.
